Office Hours:  Monday - Friday 10:00am-7:00pm.  Saturday 9:00am-1:00pm.  We accept walk-ins and appointments.

Appointments:  When a request for an appointment is made, the provider's schedule will be checked for openings.  If you have an urgent need you may be referred to a supervising physician or offered the next available appointment.  Each patient will be responsible for notifying us of any changes with: address, phone numbers, insurance, etc..  Please arrive at least 15 minutes early for your scheduled appointment.  Always bring your medications or a list of your medicines and current insurance card(s).  Please turn off your cell phones/pagers prior to your office visit.
Appointments take priority over walk-ins  unless there is an emergency situation at hand

 
Appointment Cancellations
:  Failure to cancel your appointment will result in a $5.00 fee for an office visit and $15.00 fee for a comprehensive physical examination.  Contact the office 24 hours prior to an appointment in terms of a cancellation to avoid these fees.  Any appointment cancelled on the day of the appointment will be subject to the applicable cancellation fee.

Late for the Appointment:  Please contact the office to let us know if you will be late for your appointment.  If you are more than 15 minutes late for your appointment you will be rescheduled to another time, as not to interfere with the patients arriving on time.

Prescriptions and Refill Request:  Please allow 48 hours for your prescriptions to be called into the pharmacy.  We do not prescribe narcotics at this facility. We can and will refer you to a physician for an assessment, if in need of a refill of a controlled substance.  We do not call in or fax mail order prescriptions.

Lab/Test Results:  You will be notified only for abnormal lab/test results within seven (7) days.  You will not be notified of normal results.

Telephone Calls:  The nurse will return your call within 24 hours.  Please allow one (1) week for your referral appointment to be scheduled.  An office visit may be required before making your referral.  Please allow three (3) days to obtain pre-authorization for all cat scans, x-rays, ultrasounds, etc. from your insurance company. 

Insurance Payment:  You are responsible for your co-pays and deductibles prior to being seen by the provider.  We will gladly process your claim if you have insurance.  We request that you pay your estimated portion when services are rendered. 

Physical, DOT, and other Forms:  Please allow seven (7) days for these forms to be completed.  All medical forms/documents will incur a fee ranging from $10.00 to $50.00.  This fee is expected at the time you pick up your completed forms/ documents.  No work excuse will be issued without an office visit.  

Waiting Room Etiquette:  Please limit the number of individuals accompanying you to your appointment.  No eating, drinking, or smoking is allowed in the waiting room.  Please do not wear excessive perfume, cologne, or aftershave to the office.

Privacy Policy:  Our office has adopted the HIPPA Privacy Policy as directed by the Federal Government.  This policy describes how your medical information may be disclosed.  We will not disclose any of your information without your written consent.
